Willow Lane is our community-based residential facility (CBRF) designed to support seniors in living with dignity, safety, and joy. Licensed by the State of Wisconsin, Willow Lane provides a household-style environment that fosters connection, friendship, and shared experiences. Residents enjoy a full, life-enriching lifestyle with opportunities for social engagement, enrichment activities, and personalized support, all within a caring and secure community.
At Willow Lane, our goal is to create a supportive, enriching, and convenient environment where seniors can thrive. Residents enjoy a full continuum of services all under one roof, including personal laundry, a barber and salon, interdenominational services, mail and postal services, comfortable sitting areas, and more. Our community also offers fine dining, housekeeping, linen service, maintenance, social services, and a variety of physical and life-enriching activities designed to support wellness, connection, and joy.
These 410-square-foot private apartments feature a room, a bath with a walk-in shower, and separate living/sleeping spaces. They include a kitchenette, individual climate control, extended cable, and ample safety call lights. Couples are welcome in select apartments.
Dining is a delight with our four-week rotating menu featuring seasonal, made-to-order lunch and dinner options. We easily accommodate dietary modifications, including texture-modified (mechanical soft, pureed), specialty diets (low sodium, low fat, etc.), and thickened liquids.
Residents enjoy a separate main entrance, a communal kitchen and dining area, and a cozy living room. Our amenities include a patio, outdoor courtyard, spa with a whirlpool, laundry room, and included cable and WiFi.
Willow Lane can accommodate couples in the same 410-square-foot apartment, with special reduced rates for shared living while maintaining privacy and comfort.
For resident convenience, we offer on-site physician visits and services, including Health Drive specialists (optometrist, dentist, etc.), MobileX x-rays, and an in-house EKG and bladder scanner.
Residents enjoy full-service hair and beauty treatments on-site, providing comfort and convenience without leaving the building. Appointments can be scheduled by calling (920) 458-2137 x712.
We are an all-inclusive assisted living. We have one flat monthly rate that includes all of your loved ones care needs, with the exception of transportation and the beauty shop.
Included in the monthly cost is the following: one-bedroom apartment, 3 meals/day and snacks & beverages 24/7, housekeeping, laundry services, personal hygiene items including paper products, towels, a landline, cable, Wi-Fi, medication administration, 1-2 whirlpool tub baths a week, heating /air-conditioning, 24 hour staff to assist with care needs, variety of activities including community outings.
We have 25 private apartments. They are all the same layout and size.
We offer a variety of activities ranging from worship services, bingo, Wii bowling, live music, food demonstrations, socials, happy hours, arts and crafts, exercise class, trivia and word games, resident council, card clubs, walking club and multiple community outings including lunches at local restaurants, bookworm gardens, ice cream at south pier, Meijers, Making Spirits Bright ride, etc. – all activities are included in the monthly cost.
Social services will assist with completing a Medicaid application.
The ALF has 2-3 resident assistants working first shift, 2-3 on second shift and 1 overnight. There is a nurse on the unit Monday through Friday from 8-4:30.
If a nurse is needed after hours, a nurse from the skilled nursing unit (which is attached) will come over to assist.
We do not have visiting hours. Family and friends are welcome to visit anytime.
Unfortunately we do not allow pets to live here at SSC but we love animal visits.
We have transportation available 24/7. It is not included in the monthly cost. It is $45 round trip within Sheboygan County. Outside of Sheboygan County there is additional mileage.
We do have a waitlist. To be added to the waitlist, a financial application needs to be completed. We require two years of private pay. Completing the application, which shows funds to be able to pay for at least two years privately, will allow your loved one to be added to the waitlist.

My favorite thing about Sheboygan County is that it is big and dynamic enough to have great quality of life (restaurants, entertainment, parks, etc) but still feels like a small town where everyone knows and looks out for each other.
